Creating a table, I hope, is not a problem. I will show you how to create a numeric field.

  1. Create a new text document.

  2. Enable control panel:
    View -> Toolbars -> Controls

  3. Click the button to display additional items:
    Additional controls

  4. Click the number field button and draw it (like a rectangle) in the document:
    Numeric field

  5. In the context menu open the properties of the element:
    Control ...

  6. In the window that appears, fill in the name and the required fields:
